Here are the NCHO audition entry forms. For the electronic form, please be sure to make a copy of the original before filling it out and then share your copy with me. This helps streamline the process when I copy and paste names into the schedule. Also, please double-check the spelling of your students’ names on the form—it will be a big help for us moving forward.
